Veteran Directed Care
Enrolled veterans and their caregivers can access FMS One and the Employee Self Service Portal here.
GUIDE Model
GUIDE assists eligible Medicare beneficiaries who are experiencing dementia. Bay Aging will begin serving GUIDE patients in July 2025.
